If a person appears on the 'Birthday' list for the day, and you don't feel comfortable wishing them a happy birthday, you should 'unfriend' them. I thought this was a really great idea. If you don't know someone well enough to wish them a happy birthday...you shouldn't be Facebook friends with them.
